The L/R threads allow one end to be turned in the opposite direction to the other so the overall length changes while the rod does not rotate. That makes toe changes faster and avoids disturbing connected linkages.
The 40mm denotes the nominal centre-to-centre span between ball end mounting points when the turnbuckle is at its neutral setting. Measure between the ball centres with no thread preload for an accurate figure.
Titanium resists corrosion and wears better than standard steel alloys in many RC environments, making these turnbuckles a good choice for frequent track sessions and outdoor practice.
Use a small hex driver for set screws or soft-jaw pliers for the ball ends to avoid marring the finish. Work gently to protect the threads and check adjustments with the suspension settled.
